83-year-old male patient with a history of a. fib on AC, bladder stones s/p cystolitholapaxy [12/21/2025]
and BPH with LUTS. Patient is currently taking tamsulosin 0.4 mg for BPH with LUTS without any
symptomatic improvement. Patient endorses some hematuria in the urine. States there is occasional burning
with urination and reports that sometimes the urine is not clear. Denies slow, weak stream. He endorses
urinary urgency and frequency and x2 Nocturia. Cysto/TRUS (11/2025): trabeculation severe, multiple
bladder stones, bullous edema, prostate trilobar enlargement (lateral + median), prostate volume: 234
grams. The patient has been referred by his Urologist to discuss potential minimally invasive treatment
options. The patient is accompanied by his daughter on today's televisit consultation.
Medical History: Hypertension, High cholesterol, Afib.
Surgical History: cardiac ablation , colonoscopy , dental implant , Appendectomy .

ASSESSMENT:
The patient is a male with LUTS secondary to BPH, referred by Urology for evaluation and management options.
We had an in-depth discussion of potential treatments, including surgical options, minimally invasive surgical
therapies (MISTs), and PAE. The patient is considering PAE, but would like additional time to consider prior to
proceeding.

PAE was suggested as a minimally invasive treatment option. The patient was informed that PAE works by
reducing blood flow to targeted prostatic tissue, leading to prostate gland shrinkage over 1 to 3 months post-
procedure, with an expected success rate of approximately 90% in our practice.
