Clinical Summary (HPI)

Bladder Cancer: 88yF with high-grade urothelial carcinoma s/p 2 TURBTs (no muscle in specimens). Completed 6 cycles of BCG. Presents for followup. She is currently s/p Cysto and Bladder Biopsy. Her path showed active chronic inflammation and calcification. No malignancy. She was on Solifenacin 5 mg but stopped. I had prescried Trospium. Both meds caused confusion and didn't work. She is currently off both meds. Frequency every 2-3 hrs. She denies any urgency. Occ leakage. Nocturia x 2. ========================= Previous work up: Cytology: Negative (June 2025) TURBT Path report (12/2024): Urinary blader left posterior wall, infiltrating high grade urothelial carcinoma in lamina propria. No muscularis propria present for eval. RIght posterior wall, Inflamed stromel tissue and calcifications, no mucusal epithelium identified. No evidence of malignancy. Right lateral wall bx, inflamed bladder muscosa, granulation tissue and calcifications. No evidence of malignancy TURBT 9/[ZIP]: 5cm tumor. Path HGTa UC/UA (11/2024): 100k, E.koseri/WBC 2+, RBC 25-50.
