1. Gross hematuria (ICD R31.0) without alternative explanation.
2. Microscopic hematuria (ICD R31.29) with risk factors (age over 35, smoking history, occupational exposure).
3. Confirmed bladder cancer (ICD C67.9, C67.2) requiring resection, surveillance, or adjuvant therapy.
4. Urothelial carcinoma (ICD C68.9) of any urinary tract site.
5. Bladder outlet obstruction (ICD N32.0) with diagnostic uncertainty.
6. Bladder diverticulum (ICD N32.3) with associated symptoms.

1. Urinalysis and urine cytology results.
2. Imaging report (CT urogram, MRI, ultrasound) when available.
3. Smoking history and occupational risk factors.
4. Prior cystoscopy results if performed.

6. Cancer staging (TNM) for malignant cases.
